The Paperwork Burden Reduction Act was signed into law at the end of 2024. This law affected the way you may receive the form that explains your enrollment status in the Company’s Minimum Essential Coverage (MEC) Preventive Care plan in 2025 – IRS Form 1095-C.
Most individuals do not need Form 1095-C to file their federal income tax return. However, a copy is available upon request.
If you were employed during the applicable calendar year and would like a copy of your Form 1095-C, please contact the Payroll Department.
When submitting a request, please include your full name , the calendar year you are requesting, and your phone number.
A copy will be provided no later than January 31st or within 30 days of receiving your request, whichever date is later.
